Dear Representative Team Coordinators,
The Hockey NSW Pathways and Performance Team has developed a short questionnaire to gather association input into athlete selection for our furture Pathways squads.
In consultation with your team coaches, please complete this Top 6 Player Survey to help identify athletes who may be in contention for these squads. Please note that these nominations are intended to guide selectors in identifying suitable athletes and does not guarantee selection into a Pathways squad.
While the questionnaire refers to a “Top 6” identification process, if your association has more athletes you believe should be considered, please include this information in the additional comments section. If you have fewer athletes, please select the number of athletes you wish to nominate in the form.
All form submissions must be completed one week prior to the relevant Age State Championships.
